Here I found that two subroutine in your copy are lacking:
      subroutine locr(...)
      subroutine loci(...)
In addition, the preprocessor subroutine (k3prep.f) is also lacking.
So, if any one can complete these, his/her contribution will be remarkable.

Some subroutines are useless in KIVA program if you use yourselves pre-processor and post-processor, such as ICEM-CFD and FieldView.
